To request a transcript, please fill out the following information:

NOTES:

Current seniors who plan to play a sport in college must register with the NCAA online at http://www.ncaaclearinghouse.net. Then you have to print a transcript release form from that site and bring it to the registrar with the $5 transcript fee.
AP Scores are not included on transcripts. To request the scores to be sent to a college call: 888-225-5427.
